I hereby state that all information provided is accurate and may be verified by The Janitorial Company. I agree that I may be discharged if The Janitorial Company at anytime learns of falsification or material omission in the information provided on this application form and related documents. The Janitorial Company may contact my former employers. All references are hereby authorized to release all information which they may have relevant to my employment with them. I hereby release The Janitorial Company , its affiliates, successors, and assigns, and all references from any liability that might be claimed because of information provided by such references. I agree that I will follow all policies, rules, and procedures as indicated by The Janitorial Company. I understand that The Janitorial Company reserves the right to add, change, and/or delete any policies, procedures, work rules, and/or benefits at anytime. In addition, I understand and agree that if I am employed, my employment is for no definite or determinable period and may be terminated at any time, with or without prior notice, at the option of either myself or The Janitorial Company, and that no promises or representations contrary to the foregoing are binding on the company unless made in writing and signed by me and a designated representative of The Janitorial Company. I agree to make myself available for random drug testing. If employed, I understand that I am allowed a minimum rate of a net ten consecutive minutes for each four hour work period. If employed, I understand that a rest period is not required if total daily work time is less than three and one-half hours.
